Parathyroid ultrasound: pearls and pitfalls with multimodality imaging

Imaging studies of the neck in hyperparathyroidism are important for surgical planning, as they assist in the identification and localization of one or more abnormal parathyroid glands. Ultrasound is usually the initial imaging method, and the use of appropriate transducers, technique and multimodality imaging correlation (computed tomography parathyroid protocols - 4DCT; 99mTc-sestamibi SPECT/CT and magnetic ressonance imaging- MRI) allow for appropriate diagnosis.

Discussão

Imaging assessment of patients with hyperparathyroidism in its many different settings, such as assymptomatic hyperparathyroidism, post-thyroidectomy or post-parathyroidectomy, is challenging. It demands great comprehension about cervical anatomy, normal and ectopic parathyroid locations, typical and atypical findings in parathyroid pathology and surgical procedures in the neck region.

Conclusões

Ultrasound (US) of the parathyroid glands remains one of the most important imaging studies, with different technologies such as microvascular flow imaging, high frequency transducers and elastography increasing its diagnostic accuracy. Multimodality imaging correlation and a standardized protocol for reading an ultrasound of the neck help avoid pitfalls and improve the quality of reports.
