CNS lymphomas: characteristic findings in conventional MRI and advanced sequences

Introdução e objetivo(s)

Central nervous system lymphomas (CNSL) are rare, aggressive malignancies with a bad prognosis. The incidence of CNSL has been increasing due to the rise in cases in both immunosuppressed and immunocompetent patients. As described in literature, the radiological characteristics of lymphoma mainly correspond to the most common form of presentation of the most common type, primary diffuse cell large B cells lymphoma (DLBCL). However, less frequent presentations and other lymphoma subtypes are rarely described by the World Health Organization (WHO).

Método(s)

This pictorial essay exhibits original cases of different presentations of CNS lymphoma in a tertiary center, selected by experts based on interesting traits.

Discussão

Changes in epidemiological factors, advances in imaging techniques, and recent updates of the WHO Classification of CNS Tumors in 2021 and Hematolymphoid tumors in 2022 may have made previously accepted radiological concepts obsolete. Recent studies have shown that the behavior of lymphoma is different in immunocompetent and immunocompromised patients. Likewise, primary lymphomas have different manifestations than secondary lymphomas in conventional sequences and advanced MRI techniques. Knowing their characteristic radiological patterns can guide early diagnosis, leading to presurgical management directly impacting therapeutic decisions. Another critical role may be seen in differentiating CNS lymphoma from other brain lesions, diagnostic approach, and monitoring response to treatment.

Conclusões

A completely updated pictorial review of the clinical-radiological characteristics of the full spectrum of lymphomas is useful in managing daily patient care decisions.
